Visit Unitus at Dennis Lane & Associates to enroll and to receive $3,000 of AD&D protection at no cost to you. The benefit is $1,500 if you are age 70 or over. Enrollment is required to receive this member benefit. There are no medical questions to answer and it takes less then two minutes to complete this enrollment form.

Information on an Individual Plan with higher limits or a special Family Plan are available upon request.
